The steps to apply and reserve funding for our program are outlined below.
1. Questionnaire (linked above)
2. Consent Form to Check Utility Usage (MHR 671) This will be sent in an email once the questionnaire is filled out. This is giving us consent to check the utility usage of the current home. Our program requires that someone live there within the last year.
3. Incentive Reservation Form (620 MHR) This will come after we check utilities. This document lets you know that we are reserving your funds for up to a year. Extensions are granted if needed.
4. Incentive Commitment Letter Sent to Customer. This will come after being approved for the program.
5. Payment Request Form (MHR PR) These will be the final steps to accomplish.
o All invoices/bids/sales agreements associated with costs to projects including any lending/funding documents verifying amounts
Please let us know if you have any questions and when you are ready to get started, we are here to assist every step of the way with the process. Thank you for your interest in our program.
